Bookings for NDIS Funded Patients
A deposit is not required for patients that are Plan Managed under the National Disability Support Scheme (NDIS). Instead, go to the online booking system to see availability, then fill out the form below. You will then be emailed an NDIS consent form to be completed to secure your booking.
